Abstract: No book is completed without the help and support of a whole host of people, who offer advice, encouragement, comment and criticism. Jill and Terry, like any other book would not have been finished without the support of many friends and colleagues. Foremost amongst these was my wife Karen, who provided the most consistent and deep encouragement and help, often by just being there to look after all those jobs I should have been doing and was neglecting while I was writing.
